CSI is a distinct list in SharePoint the first column of which is named 'Title' and has the entries that appear on the drop down.

The DataField entry is 'Customer_x0020_Service_x0020_Impact?'.

I am hoping that by laying out one of the two still not returning the data entered you can help me spot the problem:
